Supported Living Services (SLS)

Cottonwood provides non-licensed services and support for SLS individuals through the Community First Choice (CFC) program. We are not licensed to provide any Personal Care or hands-on care in any SLS or Community First Choice category. 

Some of the individuals we work with live with their families or on their own. Supported Living and Community First Choice Services are designed to provide them with the support they or their families need to make this a successful living arrangement. These services vary for each person, for example weekly visits from a staff member to assist grocery and personal shopping, budgeting, or time spent doing recreational activities in the community.
